Overview
We are seeking a dedicated and experienced Registered Nurse Case Manager specializing in Acute Care to join our dynamic healthcare team. This role involves coordinating comprehensive patient care for individuals with complex medical needs across various acute care settings, including hospitals, emergency departments, and post-acute environments. The ideal candidate will leverage their extensive clinical expertise to facilitate optimal patient outcomes, ensure seamless discharge planning, and promote efficient utilization of healthcare resources. Monday - Friday
Start time ranges from 8a-9a for 8.5 hours
Requirements:
2+ years of Case Manager experience required
Non acute disciplines will not be considered
Active PA RN license required
BSN Required
Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Develop individualized care plans in collaboration with healthcare professionals, ensuring alignment with clinical guidelines and patient preferences.
- Manage inpatient and outpatient cases across diverse settings such as hospital medicine, ICU, PICU, Geriatrics, behavioral health, and post-acute care.
- Coordinate discharge planning by facilitating communication among patients, families, physicians, social workers, and other healthcare team members.
- Utilize electronic health records (EHR/EHR) systems to document patient information accurately, manage medical records, and track clinical progress.
- Oversee utilization management processes to ensure appropriate level of care while adhering to health regulation policies and managed care protocols.
- Perform diagnostic evaluations and coordinate necessary interventions for patients with complex conditions such as infectious diseases or trauma cases.
- Engage in case management activities including patient service coordination, post-discharge follow-up, home health arrangements, hospice care planning, and outpatient services.
- Monitor patient progress through ongoing assessment of vital signs and clinical indicators to adjust care plans as necessary.
- Ensure compliance with HIPAA regulations and maintain confidentiality of all medical records and patient information.
Skills
- Proven experience in critical care environments such as ICU, Level I or Level II trauma centers, emergency rooms, or surgical units.
- Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques relevant to acute care settings.
- Demonstrated ability to collaborate effectively with multidisciplinary healthcare teams including physicians, nurses, social workers, and specialists.
- Proficiency in electronic health records (EHR/EHR) management systems and healthcare software applications.
- In-depth understanding of health regulation policies, HIPAA compliance standards, and managed care principles.
- Excellent patient service skills with a compassionate approach to client communication and education.
- Ability to perform comprehensive patient assessments and manage complex clinical case loads efficiently.
- Familiarity with hospital medicine protocols, discharge planning procedures, post-acute care pathways, and outpatient clinic operations.
- Experience managing diverse populations including pediatrics (PICU), geriatrics, behavioral health patients, and those requiring specialized infectious disease or trauma care.
- Strong organizational skills with attention to detail in documentation and medical records management.
