Location: Gateway + rotating support across Nephrology Clinics IU North, IU Fishers, IU Methodist, IU West, and University Hospital
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Work Arrangement: Hybrid (Must reside in Indiana)
This position supports the Kidney Care Management program by managing a patient caseload and providing outreach, education, and care coordination for individuals with chronic kidney disease (CKD). The role blends telephonic, electronic, and in-person support to improve patient engagement, outcomes, and access to care.

Conduct outreach and cold-call patients to introduce and enroll them in the program

Complete monthly check-in calls to monitor patient progress and support care plans

Perform ED follow-up calls to ensure continuity of care post-discharge

Provide patient education and coaching to support self-management and clinical goals

Assist with scheduling appointments (nephrology, imaging, transplant coordination)

Identify patient needs and coordinate referrals (e.g., Social Work for SDOH barriers)

Support patients with:

Transportation coordination
-
Visit preparation (questions, expectations)

Access to community and government resources

Document all patient interactions and maintain accurate records

Collaborate closely with a multidisciplinary care team

Participate in program evaluation and process improvement efforts
If filled by a Registered Dietitian (RD), the role will also include providing dietary education to patients.

High school diploma or equivalent required; Bachelor’s degree preferred

Experience working with patients with chronic or behavioral health needs

Experience in a multidisciplinary care setting

Experience in kidney care is preferred
