Purpose of Your Job Position
The primary purpose of this position is to implement and initiate the resident assessment instrument process for all residents of St. Mary’s Nursing Center, Inc.

Duties and Responsibilities:
· Implement and schedule resident assessments and care plans for residents admitted onto all units.
· Schedule and monitor all MDS for each unit.
· Complete MDS, Level-of-Care (LOC) and care plans for all residents as it correlates to their payor source.
· Monitor MDS and care plan completion by all disciplines and ensure completion within timeframe required.
· Monitor residents for changes in condition and initiate Significant Change of Status assessment as appropriate.
· Ensure all disciplines are aware of any changes in assessment/care plan schedules.
· Audit assessments as indicated for accuracy and compliance.
· Submit assessment information to the Health Care Finance Administration (HCFA)..
· Coordinate and lead care plan meetings with members of the interdisciplinary team, monitoring attendance.
· Complete discharge and re-admittance tracking on all residents and initiate the discharge process of MDS skill when appropriate.
· Communicate with other units when assessments/care plans are due on transfers.
· Meet daily with rehabilitation manager to set dates and verify highest minutes obtained for highest RUG/PDPM/CMI level.
· Assist Unit Managers in completing LOC, HCFA 802 and 672 forms, MDS and additional paperwork as needed.
· Follow Medicare assessment guidelines for all residents covered under Medicare Part A, ensuring highest return on reimbursement.
· Follow Medicaid assessment guidelines for all residents covered under Medicaid, ensuring highest return on reimbursement.
· Monitor Patient Driven Payment Model (PDPM) for maximum reimbursement.
· Process billing documentation for Medicare Utilization Billing and Medical Assistance (MA).
· Conduct Medicare utilization review meeting daily.
· Conduct skilled criteria review with admissions as needed.
· Review charts for discharge therapy, determine if nursing skill is needed.
· Monitor skilled charting, ensuring support of MDS.
· Monitor residents for changes in condition and need for significant change assessment.
· Coordinate MDS and care plan schedule for all transfers from rehabilitation unit to long-term care.
· Review QI/QM (quality indicators and quality measures) on a monthly basis, checking triggers for accuracy.
· Discuss and initiate with Care Plan Team the “hot triggers”, develop plans to reduce those triggers, reinforcing with the Care Plan Team the importance of documentation.
· Attend family meetings as necessary or requested.
· Provide training on care plans, MDS and ADL documentation as needed.
This position is part of the on-call rotation.
Educational Requirements
·_ Must possess a current Maryland license as a Registered Nurse. (_RN) and a current Cardio Pulmonary Resuscitation (CPR) certification.
· A minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Knowledge of Case-Mix, the Federal Medicare PDPM process, and Medicaid reimbursement. A thorough understanding of the Quality Indicator process and knowledge of the OBRA regulations and Minimum Data Set are required.
· RNAC certification preferred.
Experience
· Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long-term care as well as rehabilitative and restorative therapy practices.
· Must possess a working knowledge of the Minimum Data Set/Resident Assessment processes.
· Must be proficient with the computer.
· Exposure to care planning process.

· Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Physical and Sensory Requirements (with or without the aide of mechanical devices)
· Must be able to move intermittently throughout the workday.
· Must be able to sit and input information into the computer for long periods of time during the day.
· Must be able to cope with the mental, emotional, and physical stress of the position.
· Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirement of this position can be fully met.
· Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, support agencies and outside agencies.
· Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
· Must be able to accommodate the indicated weight (pounds) performing such duties:
- Lifting (100) -Pulling/Pushing (200) -Carrying (50-75)
· May be necessary to assist in the evacuation of residents during emergency situations.
· Must be willing to perform tasks that may involve exposure to the resident’s blood/body fluids.

Pay: $33.00 - $46.00 per hour
