Physical Therapist – Home Health
Facility: Adventist Health and Rideout
Location: Marysville, CA
Service Area: Chico/Oroville – Butte, Yuba & Sutter Counties
Contract Length: 13 Weeks
Shift: Days | Monday–Friday | 1st Shift 8hrs
Pay: $72–$74/hr
Active California PT License required
DPT required if licensed after January 2016
Active BLS/CPR Certification required
Valid Driver’s License required
Home Health experience preferred (2 years ideal)
OASIS experience preferred
Ability to travel throughout assigned counties
EMR/documentation experience preferred
Provide in-home physical therapy evaluations and treatments
Develop individualized care plans
Complete accurate clinical documentation
Educate patients and families on treatment plans
Collaborate with interdisciplinary care teams
Supervise PTAs as assigned
