About the Role
One Home Health is seeking an experienced home health nurse to support our Indiana team in a part-time capacity. Openings are available in Lake County, South Bend, and the surrounding areas.
What You'll Do-
Conduct ongoing observation, assessment, and documentation of patient symptoms and progress.

Perform initial home care visits and reassess patient needs and progress as required.

Collaborate effectively with interdisciplinary team members to ensure coordinated care.

Demonstrate proficiency in accurate and timely OASIS documentation.

Provide supervision and guidance to Home Health Aide (HHA) field staff as appropriate.

Adhere to all applicable Medicare regulations and compliance standards.

Promptly notify the physician of any changes in the patient’s condition.

Coordinate care with other disciplines to support comprehensive patient needs.

Complete all documentation and paperwork in a timely and efficient manner.

Educate patients and caregivers on disease processes, medications, care plans, and treatments in accordance with policies and procedures.

Deliver high-quality care to patients in their homes, following established policies and procedures.

Perform wound care, including assessment and treatment, as well as IV line care and maintenance.
What You Bring-
Graduate of an accredited school of professional nursing; Bachelor of Science in Nursing (BSN) or Public Health Nurse (PHN) degree preferred.

Current Registered Nurse (RN) license in the applicable state(s) of practice.

Minimum of one (1) year of experience in an acute care setting or equivalent clinical experience.

Valid driver’s license with a reliable, well-maintained vehicle; must meet state and/or agency insurance requirements and maintain a good driving record.

Current CPR certification required.
