As a per-diem social worker, your responsibilities are to: provide essential support and care for residents in a nursing home, consider the following responsibilities for a per-diem social worker, up to 16 hours per week:
- Conduct assessments to identify residents' emotional and social needs.
- Complete the MDS for resident's in a timely fashion
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Facilitate communication between residents, families, and staff to enhance care.
- Provide counseling and support to residents and their families during transitions.
- Organize and lead group activities to promote social interaction and engagement.
- Advocate for residents' rights and ensure their needs are met within the facility.
