Position Summary
The Hospice Admission Nurse is responsible for conducting hospice admissions, completing comprehensive assessments, educating patients and families about hospice services, and coordinating care to ensure a seamless transition into hospice care.
Responsibilities
- Complete hospice admission assessments and evaluations.
- Explain hospice philosophy, services, and benefits to patients and families.
- Obtain consents and complete all admission documentation accurately and timely.
- Develop and initiate individualized plans of care.
- Collaborate with physicians, case managers, social workers, and interdisciplinary team members.
- Assess patient needs and identify appropriate services and interventions.
- Ensure compliance with Medicare, state, and company regulations.
- Provide compassionate support and education to patients and families during the admission process.
- Participate in the on-call rotation as assigned.
Qualifications
- Current and unrestricted Registered Nurse (RN) license.
- Minimum of one year of nursing experience required.
- Hospice, home health, or case management experience preferred.
- Strong assessment, communication, and organizational skills.
- Ability to work independently and manage time effectively.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
