Responsibilities
Spiritual Care
- Conduct a spiritual assessment within 5 days of admission as part of the comprehensive patient evaluation.
- Assist in developing and implementing the interdisciplinary plan of care as it relates to spiritual needs.
- Provide spiritual support that honors the patient's and family's individual beliefs, religious preferences, and definition of spirituality.
- Collaborate with local clergy and faith community leaders when requested by the patient or family.
- Conduct religious services, prayer, worship, and meaningful rituals as appropriate.
- Work alongside the care team to navigate spiritual and ethical concerns that arise during patient care.
- Evaluate the effectiveness of spiritual care and adjust approaches based on patient and family response.
- Provide funeral or memorial services for patients when requested.
- Provide spiritual and emotional support to staff as needed.
Bereavement Support
- Ensure a bereavement assessment is completed within 5 days of patient admission as part of the comprehensive evaluation.
- Develop and implement individualized Bereavement Plans of Care following the patient's death, drawing on bereavement and risk assessments, IDG input, and post-death interactions with the bereaved.
- Provide ongoing bereavement support to families and caregivers for up to one year following the patient's death.
- Offer memorial services, grief support groups, community education, mailings, calls, and visits as needed.
- Provide bereavement education and consultation to IDG team members.
- Support hospice staff in coping with work-related grief and loss.
- Connect families to community resources and professional services as appropriate.
Team Responsibilities (Both Roles)
- Maintain accurate, timely clinical records including assessments, care plans, and visit notes.
- Coordinate visit scheduling weekly with Case Managers to support care team collaboration.
- Participate in IDG and QAPI meetings and quality improvement activities.
- Pursue ongoing professional development through continuing education, conferences, and professional organizations.
Qualifications
- Ordained, commissioned, or credentialed according to the practices of an organized religious group; or holds a degree in theology or from a seminary.
- Experience or education in grief counseling or loss counseling; experience counseling individuals and/or groups preferred.
- Comfortable serving in an interfaith environment with people of diverse beliefs, cultures, and lifestyles.
- Minimum 2 years of related experience preferred; hospice experience a plus.
- Valid driver's license and access to a reliable, insured vehicle in good working condition.
- Strong verbal and written communication skills.

Job Type: Part-Time
Work Location: On the road, serving patients and families in Florence and surrounding counties.
Pay: $20.00 - $25.00 per hour
