Strafford County Long Term Care Facility is seeking a full-time Physical Therapist to join our highly respected in-house rehabilitation team. Our therapy department provides skilled rehabilitation services while collaborating with nursing staff and restorative aides to maximize resident function and independence. We focus on quality outcomes, interdisciplinary teamwork, and resident-centered care.
Responsibilities
- Conduct comprehensive evaluations of residents abilities and develop individualized treatment plans.
- Provide skilled physical therapy interventions and participate in restorative programming to support resident function and quality of life.
- Collaborate with Occupational Therapy, Nursing, Social Services, Medical Providers, residents and families to promote optimal outcomes.
- Complete timely and compliant documentation, including comprehensive evaluations, progress notes, care plan development and discharge summaries.
- Excellent communication skills to effectively collaborate and participate in interdisciplinary care planning, team meetings and family conferences.
Qualifications
- Graduate of an accredited Physical Therapy program, Bachelors degree or higher.
- Current State of New Hampshire Physical Therapy license.
- Experience in long-term care, geriatric rehabilitation or related settings with 5+years preferred.
- Strong communication, clinical reasoning, and interdisciplinary collaboration skills.
- Commitment to compassionate, resident-centered care.

Pay: $49.50 - $59.00 per hour
