Overview
Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) / Medical Assistant, where your expertise will directly impact patient care and well-being. Duties
- Perform comprehensive physical examinations and vital signs assessments to evaluate patient health status.
- Administer medications, injections, and other treatments following established protocols and safety standards.
- Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures.
- Provide patient education on health management, medication adherence, and lifestyle modifications to promote wellness.
- Document all patient interactions accurately within Electronic Health Record (EHR) system we use is Medent, ensuring compliance with HIPAA regulations.
- Support triage processes by assessing urgent needs and prioritizing care for patients with acute or chronic conditions.
- Manage complex cases involving dementia care, Alzheimer's care, hospice support, or long-term care needs within assisted living or nursing home settings.
- Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute or home & community care services.
Experience
- Proven experience working in any medical setting.
- Hands-on knowledge of ICD-9/10 coding systems for accurate medical documentation and billing processes.
- Prior roles involving geriatrics and behavioral health are highly valued.
- Familiarity with EMR/EHR systems such as Medent, Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation and data management.
- Demonstrated ability to work with individuals with disabilities or developmental challenges in diverse settings like assisted living or home care environments.
- Strong understanding of anatomy and physiology principles combined with excellent patient assessment skills to deliver personalized care plans.
