Location: Jacksonville, FL
Facility: Ascension St. Vincent's Roger Main Building
Schedule: Full - Time
What You Will Do:
- Investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems.

Interview patients, assesses priorities and documents case activity.

Assist clients/families with obtaining community assistance by referral to proper resources.

Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.

Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
What You Will Need:
Licensure / Certification / Registration:
-
Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
Education:
-
High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience:
-
3 years of experience preferred.
