As a Home Health Registered Nurse, you will play a critical role in delivering skilled nursing services, monitoring patient progress, and ensuring safe, compliant, and outcome-driven care in the home setting.
Requirements
- Active, unrestricted California RN License (minimum 2 year required)
- Minimum 1 year of Home Health experience Required
- Current CPR certification
- Current TB clearance
- Current Health Screening/Physical
- Reliable transportation and valid California driver’s license & Insurance
- Proficiency in electronic documentation (WellSky experience preferred)
- Strong clinical assessment and critical thinking skills
- Ability to work independently and manage caseload effectively
- Professional, compassionate, and patient-centered approach
Responsibilities
Under physician orders and in accordance with the Plan of Care, the RN/LVN will:
- Perform comprehensive nursing assessments and ongoing patient evaluations
- Monitor and document vital signs and changes in condition
- Provide skilled nursing interventions including:
- Wound care
- Catheter care
- Drain Care
- Medication management
- IV therapy and infusion care (RN scope)
- Injections and other ordered treatments
- Educate patients and caregivers on disease management, medications, and safety
- Identify and promptly report exacerbations or changes in condition to the physician and office team
- Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
- Maintain compliance with infection control and safety protocols
- Accurately complete documentation in WellSky, including applicable visit types (SOC, ROC, Recertification, Discharge — RN scope as applicable)
- Communicate effectively with patients, families, physicians, and internal staff
