Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will work closely with our licensed personnel to provide high-quality patient care in various settings, including assisted living, home health and hospice environments. This role requires a strong understanding of patient needs, effective communication skills, and the ability to implement care plans tailored to individual patients.
Duties
- Provide direct patient care, including exercises and therapeutic modalities, under the supervision of a licensed physical therapist.
- Monitor and document patient progress, ensuring accurate records are maintained.
- Educate patients and their families on treatment plans and home exercise programs.
- Collaborate with healthcare professionals to develop effective care plans that address the unique needs of each patient.
- Utilize knowledge of medical terminology and physiology to communicate effectively within the healthcare team.
- Employ specialized techniques such as trigger point therapy when appropriate for patient treatment.
- Work with diverse populations, including geriatrics and pediatrics, ensuring a compassionate approach to care.
- Assist patients as they practice daily functional tasks by correcting form, motivating them, and ensuring their safety
Qualifications
Current state licensing for physical therapy assistance required
- Excellent communication and interpersonal skills
- The ability to create a positive, nurturing environment.
- HOME HEALTH AND HOSPICE EXPERIENCE PREFERRED
- Knowledge with Kinnser, Wellmed Documentation
