What You’ll Do
As an RN Case Manager, you’ll provide physician-ordered care in patients’ homes, including:
- Wound care and treatment management
- Medication administration and education
- Patient and caregiver education on diagnoses and recovery
- Ongoing assessment and communication with physicians and care teams
- Updating and managing individualized care plans
You’ll use your clinical expertise and critical thinking to ensure each patient receives dignified, personalized, high-quality care—all while making a lasting impact.
What We’re Looking For
- Active Registered Nurse (RN) license (Nebraska or compact state)
- Minimum 1 year of RN experience (home health, community, or med/surg preferred)
- Knowledge of OASIS (preferred)
- Current BLS/CPR certification
- Valid driver’s license, reliable vehicle, and auto insurance
