Whitney Oaks Care Center is seeking a licensed Physical Therapist (PT) to join our dedicated rehab team in Carmichael, CA. We are a skilled nursing and post-acute care facility focused on delivering high-quality rehabilitation services to help patients regain mobility, strength, and independence.

SNF or rehab experience preferred, but not required

Evaluate residents and patients and develop individualized physical therapy treatment plans

Provide skilled therapy interventions to improve mobility, strength, balance, endurance, and functional independence

Collaborate with occupational therapists, speech therapists, nurses, and physicians to support optimal patient outcomes

Educate patients and caregivers on safety, mobility strategies, fall prevention, and home exercise programs

Complete accurate and timely documentation in compliance with regulatory and facility standards

Current California Physical Therapist license required

Experience in a skilled nursing facility (SNF), rehab hospital, post-acute care, or outpatient setting preferred

Strong clinical, communication, and interdisciplinary teamwork skills
