Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Psychiatric Mental Health Nurse Practitioner (PMHNP) for Element Care PACE in Lynn, Massachusetts.
POSITION OVERVIEW
-
Provide comprehensive psychiatric evaluations, medication management, and behavioral health care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).

Develop and manage individualized psychopharmacology treatment plans while monitoring therapeutic response and medication safety.

Perform psychiatric consultations and crisis evaluations in collaboration with the interdisciplinary care team.

Provide care within PACE centers as well as participant homes and skilled nursing facilities throughout Lynn and Brighton.

Partner with Primary Care Physicians, Registered Nurses, Social Workers, Therapists, and additional specialists to coordinate participant-centered care.

Maintain accurate documentation within the electronic medical record while supporting the organization's mission of helping seniors remain safely in their communities.

Competitive salary from $130,000-$170,000 based on experience.

Requirements:
MINIMUM REQUIREMENTS
-
ANCC-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Clinical Nurse Specialist.

Active Massachusetts Advanced Practice Registered Nurse license.

Active Massachusetts prescriptive authority and DEA registration.

Minimum two years of psychiatric prescribing experience preferred.

Experience performing psychiatric evaluations, medication management, and crisis intervention.

Excellent communication and interdisciplinary collaboration skills.
-
Experience working with geriatric or medically complex adult populations is preferred.
