Job Overview
We are seeking a skilled and compassionate Nurse Practitioner (Adult Care) to join our healthcare team. In this role, you will provide high-quality care to adult patients (18+), including those with commercial insurance and HMO plans, ensuring their health and well-being through comprehensive assessments, diagnosis, and treatment. You will collaborate closely with physicians, nurses, and other healthcare professionals to deliver exceptional patient-centered care in a supportive environment.
Duties
- Perform thorough physical examinations and health assessments of adult patients.
- Diagnose and manage acute and chronic conditions across a wide range of adult health issues.
- Develop and implement individualized care plans in alignment with patients’ insurance requirements (Commercial & HMO).
- Provide preventive care, health screenings, and patient education to promote long-term wellness.
- Collaborate with multidisciplinary teams to ensure continuity and quality of care.
- Utilize electronic health record systems (e.g., Epic) for accurate documentation and patient management.
- Order, interpret, and follow up on diagnostic tests and lab results.
- Support hospital medicine practices and coordinate care transitions as needed.
- Participate in case management and quality improvement initiatives.
Experience & Requirements
- Current certification and licensure as a Nurse Practitioner (Adult or Family).
- Strong background working with commercial and HMO patient populations.
- Proficiency in basic life support techniques.
- Previous experience in primary care, internal medicine, or hospital medicine highly desirable.
- Knowledge of care coordination and insurance protocols.
- Excellent communication and patient education skills.
