Title: Case Manager (Designated Coordinator)

Hours: Monday-Friday 6:30AM-3:00PM No Weekends!

The person hired for this position will provide case management services, including compliance with documentation requirements, coordination, and implementation of individual program plans with service teams and assist with training of persons with developmental disabilities on the work floor and assist in planning and providing classroom and recreational activities.
Summary of Responsibilities :
Provide case management services including compliance with documentation requirements, coordination, and implementation of individual program plans with service teams, assist with training of persons on the work floor and personal care duties.
Case Management (Designated Coordinator) Duties:
- Coordination and evaluation of individual service delivery to facilitate the accomplishment of the outcomes according to the requirements outline in 245D and program policies
- Provide proactive interventions so as to assist persons served and prevent behavior incidents that could lead to demission.
- Work with teams to provide service to ensure customer needs (production) are met.
- Communicate effectively with teams, agency departments, and external to the agency.
- Participate as a team member in the planning and implementation of recreational classes.
- Attend and complete all trainings to meet both required training topics and hours on an annual basis. Maintain individual training record accurately.
- Exhibit appropriate work behaviors which promote development in self, co-workers, and clients.
- Provide limited personal care duties as needed as other PCA staff are often available to assist.
- Volunteer for two AAI events per year. This includes Evening Stars, Special Olympics/All Star Sporting Events, the Gala, and the Walk.
- Perform all additional duties assigned by supervisor.

Minimum Requirements:
Education/ Experience: Must meet one of the 4 education/experience qualifications to be considered for this position.
1. Baccalaureate degree in a field related to human services and one full year of full time work experience providing direct care services to persons with disabilities or persons age 65 and older.
2. Associate Degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
3. Diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
4. Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in the clauses 1 to 3 above.
Skills and Abilities:
- Have a valid driver's license and access to reliable form of transportation. Driving record must show no incidence (tickets, accidents, DWI, etc.) within the last three years. Must have proof of insurance and be insurable under AAI policy
- Must be proficient in computer skills required for job tasks (familiar with and able to use Microsoft Outlook, Word, and Excel programs).
- Knowledge of 245D licensing standards
- Able to lift up to 50 pounds
- Meet DHS criteria for background check
