LCSW or LMFT is a full-time, remote, temporary assignment with the possibility of going permanent! (PACE PROGRAM)
Position Details:
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (One late day a week, 10:00 am to 7:00 PM)
Assignment Type: Temporary with the possibility of going permanent
Location: Remote-Pico Rivera (On-site for orientation, training, and meetings as needed)
Compensation: $50 – $65/hour
Education & Required Experience:
- Professional Licensure as a Licensed Clinical Social Worker or Licensed Marriage and Family Therapist by the California Behavioral Science Board is required.
- A master 's-level degree is required.
- A minimum of 2 years currently working with the eldery, a minimum of 1 year of experience working with children, and a minimum of 2 years of clinical experience for LCSW.
- A valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross, is required.
- Bilingual Spanish is required
Position Summary:
The MSW provides the psychosocial perspective within the Interdisciplinary Team (IDT) for the PACE program. This includes participation in evaluations, assessments, care planning, ongoing services, and disenrollment processes.
Key responsibilities include:
- Conducting psychosocial assessments (scheduled and unscheduled)
- Providing individual and family counseling, education, and support
- Identifying and managing behavioral health needs
- Case management, advocacy, and discharge planning
- Facilitating family conferences and therapeutic support groups
- Coordinating advance care planning in collaboration with the PCP and IDT
- Linking participants to community resources (housing, financial aid, legal support, mental health services, etc.)
- Participating in home visits as needed
- Maintaining timely and compliant documentation
- Ensuring adherence to the Social Work Code of Ethics and HIPAA guidelines
The MSW collaborates closely with the IDT to optimize participants’ health status and overall quality of life.
Skills & Qualifications:
- Strong written and verbal communication skills
- Excellent organizational and prioritization abilities
- Proficiency in Microsoft Office (Word, Excel, Access)
- Self-motivated, adaptable, and detail-oriented
- Knowledge of DHCS, CMS, and HIPAA regulations
- Strong customer service and facilitation skills
Pay: $50.00 - $65.00 per hour
