Evaluate, diagnose, and treat speech, language, cognitive, communication, and swallowing disorders
Develop individualized treatment plans that help residents achieve their highest level of independence
Deliver evidence-based therapy interventions in a skilled nursing and rehabilitation setting
Accurately document evaluations, treatment plans, progress notes, and discharge summaries
Collaborate with nursing, therapy, dietary, physicians, and other interdisciplinary team members to achieve the best possible resident outcomes
Educate residents, families, and caregivers on therapy goals, progress, and home strategies when appropriate
Promote resident safety, independence, dignity, and quality of life through compassionate, person-centered care
Maintain compliance with all state, federal, and facility policies, procedures, and documentation standards
Active Colorado Speech Language Pathologist (SLP) license or eligibility for licensure
Graduate of an accredited Speech Language Pathology program
Strong communication, assessment, and clinical decision-making skills
A passion for serving older adults in long-term care and rehabilitation settings
