JASA operates an Assertive Community Treatment (ACT) Team dedicated to Bronx residents ages 55+ diagnosed with Serious Mental Illness (SMI). As an Older Adult Specialist, you will support clients through individual meetings and support groups—focusing on addiction recovery, coping skills, and self-advocacy—to enhance support networks through a high-intensity, team-oriented approach.

Clinical Supervision: JASA’s outpatient treatment programs provide the necessary supervision required for clinical licensure.
Specialized Impact: Provide intensive, evidence-based interventions for seniors with a history of community instability.

You will provide a range of mental health and rehabilitative services, primarily through home and community-based visits, to ensure clients remain stable and supported.
Key Responsibilities:
Clinical Interventions: Deliver evidence-based mental health interventions and support groups (including addiction recovery and problem-solving) in conjunction with the ACT team.
Field Work: Conduct frequent home and institutional visits to engage clients in their natural environments.
Assessment & Screening: Administer mental health screening tools and the Mini-Mental State Examination (MMSE) to monitor cognitive and emotional status.
Treatment Planning: Develop, implement, and document individualized treatment plans; monitor progress and adjust interventions as needed.
Referral & Advocacy: Provide appropriate information and referrals to clients and collaterals to enhance their support networks and access to resources.
Collaborative Care: Participate in regular individual/group clinical supervision, interdisciplinary team conferences, and program planning sessions.
Crisis Oversight: Rotate 24/7 on-call duties with other team members to provide around-the-clock emergency support.
Documentation: Maintain precise case records, reports, and statistics within mandated timeframes using electronic systems (EHR, Google Workspace, Microsoft Excel).
Education: Master’s degree in Social Work (MSW), Mental Health Counseling, or similar field required. International Medical Graduates (IMGs) are also encouraged to apply.
Licensure: Possession of a NYS professional license (LMSW, LCSW, LMHC, LCAT, LMFT, or Clinical Psychology) is preferred.
Experience: At least two years in a mental health setting with an older adult population preferred; experience with community visits or intensive treatment work is a plus.
Specialized Knowledge: Familiarity with comorbidities in older adults with SMI and deep knowledge of resources for aging adults.
Technical Skills: Basic computer skills and familiarity with Electronic Health Records (EHR) preferred.
