Neuro Challenge Foundation for Parkinson's is seeking a compassionate, organized, and community-focused Care Advisor to serve individuals living with Parkinson's disease and their care partners throughout Manatee County.
As a Care Advisor, you'll play a vital role in advancing Neuro Challenge Foundation's mission to improve the quality of life for people living with Parkinson's and their care partners. Serving as a trusted guide and advocate, you'll provide personalized support, education, and resource navigation while building meaningful relationships with participants, healthcare providers, and community organizations. Through compassionate care and strong community partnerships, you'll help individuals and families access the programs, services, and resources they need to navigate their Parkinson's journey with confidence.
This is a hybrid position, with time divided between community-based programs and meetings, the Neuro Challenge Foundation office, and remote work. Because many of our programs take place outside of traditional business hours, occasional evenings and weekends are required.

Neuro Challenge Foundation is dedicated to improving the quality of life of people with Parkinson's and their care partners through comprehensive care advising, education, wellness programs, support groups, and community connections. Every program and service is provided free of charge, ensuring individuals and families have access to the resources and support they need throughout every stage of the Parkinson's journey.
What You'll Do
Provide Care Advising & Support
- Deliver individualized guidance, education, emotional support, and resource navigation for people with Parkinson's and their care partners.
- Connect participants with services and resources that promote health, independence, and quality of life.
- Maintain current community resource directories and referral networks.
Build Community Partnerships
- Develop and maintain relationships with physicians, hospitals, rehabilitation providers, and community organizations.
- Collaborate with healthcare professionals and service agencies to strengthen the Parkinson's care continuum.
- Identify and cultivate new community partners, presenters, sponsors, volunteers, and supporters.
Facilitate Programs & Community Engagement
- Lead support groups, educational workshops, and community programs.
- Represent Neuro Challenge Foundation through community outreach, public speaking engagements, and special events.
- Support fundraising initiatives and organizational events.
Documentation & Reporting
- Maintain accurate documentation of participant interactions and services.
- Track attendance, demographic information, and program outcomes.
- Complete monthly reports and maintain shared calendars and communication tools.
Professional Development
- Participate in ongoing education related to Parkinson's disease, caregiving, community resources, and emerging treatment approaches.
- Attend conferences, webinars, and other professional development opportunities.
Qualifications
Required
- Two to three years of experience in healthcare, social services, nonprofit, or a related field involving direct client or patient interaction.
- Excellent communication and relationship-building skills.
- Strong organizational and time-management abilities with the capacity to manage multiple priorities independently.
- Experience maintaining confidential records and documentation.
- Proficiency with Microsoft Office Suite, Google Workspace, database or client management systems, and the ability to effectively use virtual communication platforms such as Zoom for meetings and program delivery.
- Ability to work occasional evenings and weekends.
- Reliable transportation and willingness to travel throughout the service area.
- Ability to lift up to 25 pounds for program and event support.
Preferred
- Bachelor's degree in Social Work, Human Services, Health Education, Nursing, Psychology, Public Health, or a related field.
- Experience facilitating support groups, educational programs, or community outreach.
- Knowledge of Parkinson's disease, caregiving, chronic disease management, behavioral health, or community-based health programs.
