Visiting Counselors of New York is hiring Licensed Clinical Social Workers (LCSWs) to provide in-home and telehealth psychotherapy to older adults throughout Suffolk County.
This is an excellent opportunity for clinicians who want meaningful clinical work, real schedule flexibility, and the ability to build a caseload that fits their life. Clinicians may provide sessions in clients’ homes, or through a combination of both in-home and telehealth based on client needs, referral availability, and clinician preference.

About the Work
Our clinicians provide psychotherapy to older adults who may be experiencing depression, anxiety, grief, bereavement, caregiver stress, chronic illness, social isolation, adjustment to aging, medical challenges, family stressors, or major life transitions.
Many of the clients we serve benefit from care that is flexible, accessible, and compassionate. For some clients, in-home therapy provides access to care they may not otherwise receive. For others, telehealth offers the convenience and consistency they need to remain engaged in treatment.
This is meaningful clinical work with a population that is often underserved and deeply appreciative of consistent, high-quality care.
Responsibilities
- Provide individual psychotherapy in clients’ homes and/or via telehealth
- Complete comprehensive clinical assessments
- Develop individualized treatment plans and treatment goals
- Provide ongoing therapy and update treatment plans as clinically indicated
- Maintain timely and accurate documentation in TherapyNotes
- Provide referrals and community linkage when appropriate
- Collaborate with family members, physicians, facilities, care managers, and other supports when clinically appropriate and authorized
- Maintain professional, ethical, and clinically appropriate care standards
Required Qualifications
- Active New York State Licensed Clinical Social Worker (LCSW) license
- Strong clinical assessment and treatment planning skills
- Ability to work independently and manage your own schedule
- Basic comfort using internet-based systems and online documentation platforms
- Reliable transportation if providing in-home appointments
- Professional, responsive, and organized communication style
Preferred, But Not Required
- Experience working with older adults
- Experience with depression, anxiety, grief, bereavement, caregiver stress, chronic illness, cognitive changes, adjustment to aging, or life transitions
- Prior outpatient, in-home, telehealth, geriatric, medical social work, skilled nursing, assisted living, hospital, care management, hospice, or private practice experience
- Bilingual clinicians are encouraged to apply
Schedule
Clinicians choose their own availability and caseload size but agree to maintain a minimum caseload size of 4 clients. This position can be a small supplemental caseload, a larger part-time schedule, or a fuller caseload depending on clinician preference and referral availability.
