Shelter Case Manager
Halo Network is seeking a full-time Case Manager, who will report to the Program Manager and is responsible for conducting intake and assessments, developing and implementing Independent Living Plans, linking Families with community-based service resources.
The Case Manager is responsible for maintaining regulatory compliance and overall site operations. The Case Manager will monitor and supervise the Support Worker assigned to the shelter.
Core Responsibilities
- Managing the operation of Family shelter
- Ensures a safe and secured environment.
- Perform intakes for new clients upon arrival to shelter
- Negotiate conflict between clients if the occasion arises
- Case Manager will increase clients’ self-sufficiency by through goal planning, assisting with employment and housing, as well as, medical, substance abuse and or mental health resource linkages.
- Case Manager documents client demographic information, all interactions with clients and outside providers in a timely manner, including maintaining accurate, up to date client files.
- Planning and providing training and supervision of Support Worker’s through staff meetings and individual sessions, or as needed based upon performance.
- Monitoring and maintaining high standards of physical safety in the shelter unit as evidenced by monthly inspection reports, accident reports, investigation reports and compliance with fire safety regulations.
- Completing supervision and disciplinary notes, and performance evaluations
- Attend mandatory training.
- Reports/Paperwork: Ensure completion/submission of all required daily, bi-weekly, weekly, monthly, quarterly and annual reports as scheduled.
Schedule: Tuesday through Saturday 8am -4pm ( with (2) evening shifts 4-12am)
QUALIFICATIONS:
Associate’s degree or equivalent required. or Bachelor's degree and/or 1-year experience in human services is preferred. Candidate must possess the ability to problem solve, make clear decisions and work as part of a team. Excellent interpersonal and verbal and written communication skills required. Proficient computer skills, including Microsoft Office required. Candidate must possess a valid New York State Driver’s License and a history of safe and responsible driving.

Pay: $26.00 - $29.00 per hour
