Department: Care Management
Location: Morristown, NJ 07960
Schedule: 8:00 AM – 4:00 PM
Hours: 37.5 hours/week | 5 days/week
Weekend/Holiday: Rotating weekends every 4th or 5th weekend; 1 summer and 1 winter holiday
Position Summary
The Case Manager is responsible for coordinating patients throughout their acute-care hospitalization, ensuring appropriate utilization of healthcare resources and facilitating safe, timely transitions of care.
This role manages all aspects of discharge planning, care coordination, utilization management, and quality screening while collaborating with physicians, nurses, patients, families, and interdisciplinary teams. Inpatient Case Management experience is required.
Key Responsibilities
- Coordinate patients throughout the acute-care hospitalization.
- Manage the complete discharge planning and transition-of-care process.
- Assess patient needs and develop individualized discharge and care plans.
- Coordinate appropriate post-acute services, resources, referrals, and follow-up care.
- Perform utilization management and quality screenings in accordance with organizational and regulatory standards.
- Monitor patient progress and identify barriers to discharge.
- Collaborate with physicians, nursing staff, social workers, therapists, payers, and other members of the interdisciplinary care team.
- Facilitate communication among patients, families, providers, and post-acute care organizations.
- Evaluate the appropriateness and effectiveness of care plans and make recommendations as needed.
- Support efficient use of healthcare resources while maintaining quality patient outcomes.
- Assist with transitions between acute and sub-acute/post-acute settings.
- Identify potential discharge barriers and proactively develop solutions.
- Maintain accurate and timely documentation of case management activities.
- Ensure compliance with hospital policies, regulatory requirements, and case management standards.
- Advocate for patients and help ensure continuity of care throughout the hospitalization and after discharge.
Required Qualifications
Education
- Bachelor of Science in Nursing (BSN) required.
Licensure
- Current and valid Registered Nurse (RN) license required.
Experience
- Inpatient Case Management experience is required.
- Experience with acute-care patients strongly preferred.
- 2+ years of acute-care experience strongly preferred.
- Experience with sub-acute patients required.
- Experience with case management and managed care preferred.
Preferred Certification
- CCMC (Certified Case Manager) preferred.

Ideal Candidate
The ideal candidate is an experienced RN Case Manager with a strong inpatient background who understands acute and sub-acute patient needs and can independently manage discharge planning, utilization management, and transitions of care. The candidate should be comfortable collaborating across multidisciplinary teams and navigating complex discharge and managed-care situations.
Pay: $50.00 - $58.00 per hour
