We are seeking Physical Therapists Assistants (PTA)to join our team of home health professionals. The successful candidate will be responsible for providing quality care to patients in their homes, assessing and monitoring patient conditions, and providing education and support to patients and their families. The ideal candidate for this position will have excellent communication skills, an ability to work independently, and a commitment to providing the highest level of care.
Requirements:
- Graduate of an accredited Physical Therapist Assistant (PTA) program
- Current Illinois Physical Therapist Assistant license in good standing
- Minimum 1 year of clinical experience (home health experience preferred)
- Current BLS/CPR certification
- Valid Illinois driver's license
- Reliable transportation with current automobile insurance
- Ability to travel throughout assigned service area
- Strong communication and interpersonal skills
- Ability to document accurately using an Electronic Medical Record (EMR)
- Ability to work independently while collaborating with the supervising Physical Therapist
Essential Functions:
As a Physical Therapist Assistant you will:
- Provide physical therapy interventions according to the established plan of care developed by the supervising Physical Therapist.
- Report patient progress, changes in condition, and concerns to the supervising PT.
- Document visits accurately and complete notes within agency timelines.
- Educate patients and caregivers on exercises, mobility, fall prevention, and use of assistive devices.
- Maintain compliance with agency documentation standards.
- Participate in interdisciplinary team meetings as appropriate.
- Maintain patient confidentiality and professional standards.
