Position Purpose:
The primary purpose of this position is to plan, organize, develop, market, and direct the operations of Living Well Senior Solutions (LWSS) within an ERS designated market area in accordance with current federal, state, and local standards, guidelines, and regulations. Provide Aging Life Care™ management to clients and their families as they navigate through the complex issues associated with aging. Ensure that all services provided to clients are in accordance with ERS’ standards and at or above ERS’ expected levels of client satisfaction.
Minimum Qualifications:
- Bachelor’s degree in social work (BSW or MSW preferred), nursing, or health care related field with a current Ohio license.
- Strong, creative leadership skills with an entrepreneurial spirit and proven track record.
- Minimum of 5 years’ experience in a position with similar responsibilities. Experience in Aging Life Care management and/or case management preferred.
- Extensive knowledge of older adult needs and support and programs available.
- Willing to work very flexible hours including being available on call 24/7 according to client needs and team member coverage.
- Ability to cope with the mental and emotional stress of the position and to remain calm and thoughtful in assisting other team members in maintaining their professionalism.
- Ability to develop an excellent rapport with agency referral sources.
- Ability to provide insight regarding difficult clients and complex problems and oversight in the supervision of other professionals who are handling difficult client situations.
- Strong computer skills required with high proficiency in Microsoft Office as well as payroll and billing documentation systems.
- Good understanding of office equipment functions (i.e., copier, printers, fax, etc.)
- Excellent organizational and problem-solving skills with strong attention to detail and time management.
- Adaptable and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, clients, family members, and care providers.
- Able to read, write, and understand the English language and speak the language in an understandable manner.
- Possess sight/hearing sense or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
- Must be able to maintain regular attendance; work will be performed at the office and onsite at various locations depending on client needs.
- Must be able to stand, sit, bend, stoop, lift, and push up to 25 pounds on a regular basis.
- Must be fully up to date with required vaccinations.
As the Director of Living Well Senior Solutions you can expect to:
- Plans, develops, and organizes the LWSS programs and activities.
- Provides care management to clients according to established guidelines.
- Provides leadership and supervision to care managers and staff. Assists in the recruitment, interviewing, selection, counseling, and professional development of staff to ensure that all staff perform their responsibilities following ERS’ expected behaviors and established policies and procedures at all times.
- Assigns clients to the appropriate team members and assures team members are readily available to meet the needs of clients. This includes 24/7 availability 365 days per year. Provides back up to care managers as required to meet client needs.
- Partners with CCRC admissions teams to provide LWSS Transition Support Package. Offer guidance, support, and assistance for out-of-town resident move-ins as needed.
- Maintains oversight of credit card reporting and approves team member purchases.
- Maintains effective relations with clients, families, team members, referral sources, clients’ physicians and outside community health, welfare and social agencies, and others as need to provide effective services to meet client needs.
- Interacts, supports, and develops plans with other departments (CCRC Health Services, Meals on Wheels, Center for Memory Support and Inclusion, Middle Market, Fund Development, etc.) to promote and integrate services.
- Completes intake of inquiries, follow up, documentation, and sign up of new clients after they have committed to services. Maintain data on referrals and discharges.
- Completes billing and assure accuracy of unit billing, associated paperwork, and customer receipt. Monitors accurate input of staff billing hours and sends out client billing timely on a monthly basis.
- Manages Personal Emergency Response System (PERS) system through Valued Relationships, Inc. (VRI). Orders, installs, and returns equipment as needed. Maintains client records and assure accurate billing.
- Completes and approves biweekly payroll for LWSS team members.
- Completes annual performance evaluation for LWSS team members.
- Participates in the development of and routine reviews of program’s policies and procedures.
- Develops, implements, and maintains an ongoing quality improvement program for the department including client and family satisfaction.
- Partners with VP of Middle Market and Ministry to developing LWSS annual objectives. Manages departmental budget and submits quarterly management reports as directed by VP.
