This is a high-level Designated Coordinator position that requires additional education and experience, with strong leadership and development competencies to guide teams and drive performance. This role provides support, planning, and engagement to individuals with various and sometime challenging maladaptive behaviors.

Schedule:
Monday- Friday 8:00am- 4:00pm
Rotating On Call Responsibilities
Expectations for a Designated Coordinator:
-
Knowledge about current licensing standards/requirements to ensure compliance with the program they are responsible for
-
Ensures compliance with each person’s Coordinated Service and Support Plan (CSSP)
-
Ensures the duties of the direct support staff are fulfilled according to responsibilities in the CSSP and CSSP Addendum.

Evaluate service delivery methods and progress toward each person’s outcomes using measurable, observable criteria, and ensure outcomes remain appropriate and person-centered.

Provide continuous training to the direct support staff

Update team members for each person served at minimum of once per month about status of service, quality of care, person situation and outcomes

Responsible for ongoing communication with team members. This included a 90 day and annual reviews
-
Other duties as assigned.
Physical Requirements:
Must be able to assist residents with mobility, transfers, lifting, and personal care tasks. Position requires prolonged standing, walking, repetitive movement, manual dexterity, and the ability to respond to emergencies. Must be adaptable to varying care environments while ensuring resident safety and support.
Required Qualifications:

Must have a valid Driver’s License

Must be willing to complete a Human Services background check and Motor Vehicle Record search

Must be available for On-call responsibilities

Required 16 hours of direct support

Must have previous experience and or Education with mental health for at least 3 years. Behavioral health is Preferred

Knowledge and understanding Positive Support Strategies and effective de-escalation techniques
Preferred Qualifications:
- Proficiently use Microsoft Office

Knowledge with 245D services/paperwork

Management Experience

Familiar with EUMR'S
Required Experience/Education

No degree – min. 6 years’ experience with positive behavioral supports or,

Associates – min. 5 years’ experience with positive behavioral supports or,
-
BA – min. 4 years’ experience with positive behavioral supports

MA – min. 3 years’ experience with positive behavioral supports
