CarePath Behavioral Health , formerly Southwest Alabama Behavioral Health Care, is a public non-profit community health center dedicated to assisting individuals on their mental health journey. The ideal care coordinator candidate will provide case management services to incarcerated adults with possible mental illness who are in current/immediate need of mental health services. The care coordinator will assist with the initial screening of individuals to help determine appropriateness for mental health services.
Services are initiated and provided primarily in the jail setting but could also be provided in the community, outpatient or home settings. While the individual is incarcerated the care coordinator will assist in securing mental health services. This could include but is not limited to coordinating telehealth services, meeting face to face the consumer to discuss needs and coordinating with other team members regarding progress, concerns, and needed services.
As the individual prepares for release the care coordinator will assist individuals with securing social security benefits if appropriate, arranging housing/rental assistance , accessing additional mental or regular health services and coordinating appointments with other mental health professionals at a CarePath outpatient office or with other mental health centers in the home area of the individuals release.
This care coordinator position is designed to help decrease the recurrence of incarceration for those individuals who simply need support on their mental health journey to help them move forward on their life path.
