PIE Management is seeking an experienced and compassionate Registered Nurse (RN) to provide comprehensive nursing assessments, care coordination, and health advocacy services for older adults and individuals with complex healthcare needs.
This position focuses on geriatric case management, person-centered care planning, and community-based health services. The ideal candidate will possess strong clinical assessment skills, experience working with aging populations, and a commitment to improving the quality of life for vulnerable community members.
Conduct comprehensive face-to-face nursing assessments, including Nursing Facility Level of Care evaluations and wellness assessments.
Obtain and document medical histories, vital signs, health status information, and clinical observations.
Develop, implement, and monitor individualized care plans in collaboration with participants, caregivers, physicians, and service providers.
Reassess participant needs and adjust care plans as health conditions and circumstances change.
Manage an assigned caseload of participants requiring ongoing clinical oversight and support.
Coordinate healthcare services and community resources to promote independent living and positive health outcomes.
Conduct regular wellness checks, follow-up assessments, and participant monitoring.
Collaborate with physicians, specialists, hospitals, home health agencies, and community-based organizations to ensure continuity of care.
Provide health education, disease management support, and advocacy for participants and caregivers.
Assist participants in navigating healthcare systems and accessing available benefits and services.
Support individuals with chronic medical conditions, dementia, behavioral health concerns, and age-related health challenges.
Participate in discharge planning and transitions of care when appropriate.
Maintain accurate, timely, and HIPAA-compliant documentation within electronic health record and case management systems.
Complete assessment reports, care plans, progress notes, and required program documentation.
Ensure compliance with federal, state, and agency regulations governing healthcare and aging services.
Participate in quality assurance and utilization management activities.
Follow infection control, patient safety, and organizational policies and procedures.
Support continuous improvement initiatives focused on participant outcomes and service quality.
Experience working with older adults, chronic medical conditions, geriatric care, dementia care, or community health programs preferred.
Experience in case management, care coordination, utilization review, or discharge planning is highly desirable.
Valid Michigan Driver's License.
Reliable transportation and ability to travel throughout the service area.
Proficiency with Microsoft Office and electronic medical record systems.
Strong understanding of geriatric nursing principles and chronic disease management.
Experience with acute care, post-acute care, behavioral health, and community-based healthcare services.
Knowledge of medical terminology, physiology, and clinical assessment techniques.
Familiarity with HIPAA regulations and healthcare documentation standards.
Excellent communication, critical thinking, and organizational skills.
Ability to work independently while collaborating effectively with multidisciplinary teams.
Experience conducting comprehensive patient assessments and developing individualized care plans.

This position is primarily community-based and requires travel to participant homes, healthcare facilities, and community locations throughout the Detroit service area.
