Innovative Health Care Concepts (IHCC) is seeking energetic,
self-motivated individuals for our Targeted Service Coordination program
in our Idaho Falls office. Services Coordinators provide information and referral services to eligible participants and their families, as well as develop local resources and helping developmental disability agencies to address the needs of eligible participants and their families. This position requires frequent contact with local helping agencies and works with individuals and family members who may be in crisis or extreme distress. Successful Service Coordinators are frequently self-starters, receptive to feedback/correction, and are people oriented. The successful candidate is someone who is a creative problem solver, can manage diverse relationships, is action oriented, communicates effectively and is organized.
This is a professional level position working closely with people with
intellectual and developmental disabilities. Our current opening can be
full-time or part-time with an opportunity to grow.
Applicant must have at least a Bachelor's degree in a human services
field, Valid Idaho Driver's License with a reliable automobile that is insured in accordance with state requirements and have an acceptable driving record, and able to pass a criminal history and background check. . Previous experience working with individuals with developmental disabilities is preferred but not required.
Hourly wage is paid per billable hour
Full-Time is 30 Billable hrs/wk
Part-time is 10 - 29 Billable hrs/wk
Training wage: $22/hr
Billable hour wage: $23 - $26/hr depending on experience
Job duties include:
Monitoring and coordinating services as requested
Developing and writing service plans and assessments
Participating in monthly team meetings with other providers
Serving as liaison for individual/family
Documenting services provided and completing required paperwork on time
and accurately
Traveling to client homes and provider agencies
Attending monthly staff meetings and trainings
