Harmony Home Health seeks a Full Time or Per-Diem Licensed Vocational Nurse to join its dynamic team of healthcare professionals in the San Francisco, CA. Harmony Home Health is Medicare-Certified and State-Licensed.
The responsibilities of the Licensed Vocational Nurse include:
- Provides direct patient care as defined in State’s Name Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
Minimum qualifications of the Licensed Vocational Nurse include:
- Active Licensed Vocational Nurse License in the state of California - at least 1 year professional LVN experience required
- Home Health (LVN) experience is a plus
- Able to prioritize and manage time effectively.
- Highly organized and detail-oriented.
- Able to follow verbal and written instructions
- Able to communicate effectively. Bilingual communication a plus, Cantonese/Mandarin.
- Dependable Transportation, Current Driver’s License and auto Insurance.
- Current CPR
- Ability to perform the physical aspects of the position.
- Complete TB test and Physical Examination
