Overview
Join our compassionate hospice team as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN), where your expertise will make a profound difference in the lives of patients and their families during their most vulnerable moments. Responsibilities
- Conduct comprehensive patient assessments to evaluate physical, emotional, and psychological needs, utilizing advanced nursing skills and medical terminology knowledge.
- Administer medications, including pain management and symptom control, following HIPAA guidelines and aseptic techniques to ensure safety and efficacy.
- Develop and implement individualized care plans aligned with patients’ goals, working within NCQA standards and ICD coding protocols for accurate documentation.
- Provide emotional support and education to patients and families about disease progression, medication administration, tube feeding, and end-of-life choices.
- Collaborate with interdisciplinary teams to coordinate care across home & community settings, ensuring seamless discharge planning and case management.
- Monitor vital signs, perform physical examinations, manage airway and ventilator needs, and respond promptly to acute pain or emergent symptoms.
- Maintain detailed medical records using EMR/EHR systems such as Epic or Cerner while adhering to HIPAA regulations.
Qualifications
- Valid RN or LVN license with current state certification; additional certifications in Hospice & Palliative Medicine are preferred.
- Critical care experience or ICU background is highly valued for managing complex symptom control in end-of-life scenarios.
- Experience working with individuals with disabilities, memory care needs, or in long-term care settings such as nursing homes or assisted living facilities.
- Proficiency in medical documentation, ICD-10 coding, medication administration, IV infusion techniques, phlebotomy, suturing, and aseptic technique.
- Strong knowledge of physiology, anatomy, pharmacology related to pain management and symptom control; familiarity with laboratory testing is a plus.
- Excellent communication skills for triage assessments, patient education, family coaching, and interdisciplinary collaboration.
- Ability to work independently in home & community care environments while maintaining high standards of infection control and patient safety. Pay: $30.00 - $100.00 per hour
