Social Services Designee
We are currently accepting applications for a Social Services Designee within our long-term care and rehabilitation facility specializing in geriatric care.
We are seeking a dependable, compassionate, and organized individual who enjoys working closely with residents, families, and interdisciplinary team members. This position plays an important role in supporting the emotional, social, and psychosocial needs of our residents while helping ensure quality of life throughout their stay.

Desired Qualifications

Licensed Social Worker in Alabama NOT required

Education in Social Sciences, Social Work, or a related field preferred

Long-Term Care or Healthcare Experience preferred

Essential Job Functions

Assist in planning, organizing, implementing, and evaluating the facility’s Social Services program.

Serve as an advocate and liaison for residents and their families regarding social, emotional, and psychosocial needs.

Assist residents and families with adjustment to the facility and changes in condition or level of care.

Identify and address medically related social and emotional needs of residents on an individual basis.

Communicate and collaborate with nursing, therapy, administration, and other members of the interdisciplinary care team.

Maintain accurate and timely documentation in accordance with facility policies and applicable regulations.

Assist with care planning and discharge planning as appropriate.

Help connect residents and families with available community resources and services when needed.

Ensure Social Services practices remain in compliance with applicable federal, state, and local regulations.

Perform additional Social Services duties as directed by the Administrator and Social Services Director.

Complete all required resident assessments, screenings, and social services documentation accurately and within required federal, state, and facility timeframes.
