$23.00 – $27.00/hour (DOE) + Mileage Reimbursement
Signature Albany Home Health & Hospice is seeking a dedicated Home Health & Hospice Aide (HHA/CNA) to provide essential personal care and support services to patients in their homes.
This PRN (as-needed) role is ideal for caregivers who are passionate about patient-centered care, hospice support, and improving quality of life for individuals facing illness, recovery, or end-of-life needs.

Assist patients with personal care, including bathing, grooming, hygiene, and dressing

Support toileting, mobility, and transfers

Provide oral care, skin care, and repositioning to prevent breakdown

Assist with meal preparation, feeding, and light housekeeping
-
Perform range of motion exercises and activity support per plan of care

Observe and report changes in patient condition to the RN or therapist

Take and document vital signs (as trained and permitted)

Maintain accurate, timely documentation of care provided

Work under supervision of a Registered Nurse (RN) or therapist

Participate in the interdisciplinary care team (IDG)

Attend case conferences and in-service training
-
Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) in Oregon

Completion of a state-approved training and competency program

Minimum age of 18 years

Current CPR certification

Valid driver’s license, reliable transportation, and auto insurance

Ability to work independently with minimal supervision

Ability to follow care plans and document accurately
