Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals.
- Administer medications and perform catheter care following medical terminology guidelines and home healthcare regulations.
- Document patient care accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA and maintaining patient confidentiality.
- Support clients with meal preparation, light cleaning, laundry, and other home care tasks to promote a safe and comfortable environment.
- Provide specialized dementia and Alzheimer's care, including memory care techniques and end-of-life support in hospice settings.
- Communicate effectively with patients and their families to ensure understanding of care plans and foster trustful relationships.
Qualifications
- Valid CNA certification or Home Health Aide license required; experience working with individuals with disabilities or in geriatrics is highly preferred.
- Demonstrated experience in assisted living or nursing home environments is advantageous.
- Knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS/CPR) is essential.
- Familiarity with home health care assistance, hospice care, dementia care, and working within home health agency regulations is desirable.
- Ability to lift heavy objects safely and perform patient transfers using proper techniques such as Hoyer lifts or other transfer methods.
- Strong communication skills for client interaction and documentation; experience working with EMR/EHR systems is a plus.
- Commitment to maintaining patient confidentiality under HIPAA standards; experience working with geriatric patients preferred.
