Case Manager/Independent Support Coordinator for the Clarksville area, working with individuals with intellectual and/or developmental disabilities, assisting the individual in identifying, selecting, obtaining, coordinating, and using both paid services and natural supports to enhance the person's independence, integration in the community, and productivity as specified in their plan. The position will be home based after receiving training in-house. Competitive benefits, monthly incentive. Computer skills are a must.
Education requirement:
1. A bachelor's degree from an accredited college or university in a human services field.
2. A bachelor's degree from an accredited college or university in a non-related field plus one year of relevant experience.
3. An associate degree plus two years of relevant experience.
4. High school diploma or general educational development (GED) certificate plus four years of relevant experience.

Pay: $39,059.45 - $42,059.45 per year
