Seeking Full-time Licensed Social Worker/LMHC/CASAC as a Senior Addiction Counselor for Intensive Outpatient and Clinic program at the Maxwell Institute. Provide comprehensive assessments, evaluations and diagnostic summaries for chemically dependent and dually-diagnosed patients. Develop patient-centered, individualized and comprehensive treatment planning. Document patient progress towards goals in treatment within OASAS and other regulatory standards. Facilitate lectures and provide group therapy. Provide case management services and coordination of care with key stakeholders. Complete discharge summaries and appropriate aftercare planning. Excellent written, computer and oral communication skills necessary. Potential candidate will be able to work well within the framework of a multi-disciplinary team. Experience with dually-diagnosed and/or adolescent populations preferred. Great location. Position requires Qualified Health Professional: CASAC, LMHC, LCSW; LMSW preferred.
