VNA of Ohio is one of Northeast Ohio's most established home health agencies - 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're building our PRN Palliative Care LPN team and offering competitive per-visit pay with a flexible schedule.
Per-Visit - $42
Downtime (pre-approved wait time) - $30/hr
"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.
Cleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand - with a flexible schedule.

Provide direct patient care to palliative care patients in their homes in accordance with the established plan of care

Monitor and document patient symptoms, comfort levels, and changes in condition

Administer medications and treatments as directed by the RN Case Manager or physician

Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals

Provide education and emotional support to patients and families managing serious illness

Maintain accurate and timely documentation in the EMR within required timeframes

Must-Have:

Active LPN license (Ohio) + current BLS certification

Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area

Minimum 1 year of clinical nursing experience
Preferred / Nice-to-Have:

Prior palliative care, hospice, or home health experience

Experience supporting patients and families through serious illness

Familiarity with HCHB or similar home health EMR

Strong communication and family support skills
