We are seeking a dedicated Young Adult Case Manager to support individuals ages 18–26 living with mental health challenges and co-occurring needs. In this role, you will help young adults build independence, develop life skills, increase stability, and successfully navigate the transition into adulthood.
Services are community-based and person-centered, meeting individuals where they are and providing support at times and locations that best meet their needs. This position maintains a smaller caseload to allow for more frequent contact, individualized support, and stronger relationship-building with recipients.
Responsibilities
- Provide community-based case management services to young adults ages 18–26 experiencing mental health and/or co-occurring challenges
- Support recipients in developing independence, emotional stability, and community integration
- Assist young adults with navigating transitions related to housing, education, employment, healthcare, and daily living skills
- Coordinate person-centered services that connect recipients to medical, social, educational, vocational, and community resources
- Maintain frequent contact with recipients through community visits, skill-building activities, and ongoing support
- Assist recipients in identifying and developing natural supports within their community and personal relationships
- Foster an environment of acceptance, empathy, hope, and empowerment while promoting recovery and improved quality of life
- Provide services primarily in the community, based on recipient preferences for time and location
- Collaborate with internal providers, families, community partners, and other support systems to ensure coordinated care
Education
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field required
Licensure
- Professional licensure preferred, but not required
- CAAC (Certified Advanced Alcohol and Drug Counselor) credential preferred
- If licensed or certified, credentials must be valid and in good standing
Experience
- Minimum of 1 year of experience in community mental health, case management, or a related human services field preferred
- Experience working with adolescents, young adults, or transitional-age youth strongly preferred
