Summary
We are seeking a dedicated Personal Care Aide / Home Health Aide to join our team in providing exceptional care to our clients. NYSDOH licensed Assisted Living Facility in Hawthorne, NY, this role is vital in supporting our mission of enhancing the quality of life for those we serve by delivering personalized assistance and support.
Responsibilities
- Provide compassionate personal care and assistance with daily living activities (ADLs) for clients.
- Monitor and document patient observations, reporting any changes in condition.
- Assist clients with mobility, including transferring and positioning.
- Maintain a clean and safe environment for clients, including laundry and light housekeeping tasks.
- Collaborate with healthcare professionals to develop individualized care plans.
- Foster positive relationships with clients to enhance their emotional well-being.
Qualifications
- Certification as a Personal Care Aide or Home Health Aide.
- Proven experience in personal care or home health settings.
- Strong understanding of patient care principles and practices.
- Ability to assist with ADLs effectively and compassionately.
- Knowledge of dementia care practices is a plus.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a team.
- CPR certification is preferred.
