American Traveler is seeking an experienced RN for a Case Management position in Key West, FL, requiring acute care case management experience and an active FL or compact RN license.

Hospital-based acute care setting in Key West, FL

Case Management unit

Day shift schedule, 4x10-hour shifts per week (Saturday through Tuesday), core hours 8:00 AM – 4:00 PM

13-week contract

Involves medical necessity reviews for Medicare, non-commercial payers, and Observation patients regardless of payer source
-
Requires completion of 3008 forms, PASRR screenings, and referrals to SNF, HHC, and ARF
-
Applies knowledge of Medicare Conditions of Participation, including the 2-midnight rule and 3-day stay requirement for SNF discharges
-
Active FL state RN license or compact multistate RN license required (active, not pending)

Current BLS certification required (must not expire within the first 30 days of the assignment start)

Minimum 1 year of recent case management experience in an acute care hospital setting

Ability to independently perform medical necessity reviews and discharge planning for patients of all ages
-
No gaps in work history of 90 days or more within the past 12 months

Must not have worked at any CHS facility as permanent staff within the past 2 years

Candidates must reside more than 50 miles from the facility to be eligible for this position

Housing secured for the full duration of the assignment required at time of acceptance

Collaborates with the interdisciplinary healthcare team to manage facility resources, reduce physician practice variations, and improve patient outcomes with a focus on decreasing length of stay
-
Responsibilities include determining medical necessity for admissions and continued stays, identifying inappropriate or untimely services, coordinating discharge planning, and facilitating timely and appropriate discharges

Identifies opportunities for improvement in patient care and maintains required regulatory documentation

RTO requests must be for less than one week in duration
