Scanning
- Daily retrieval of all hospital departments paper medical records and verify scanned into electronic medical record.
- Responsible for scanning and indexing all inpatient, outpatient, extended care and emergency room patient reports/documentation into appropriate patient’s electronic medical record and storage of paper record.
Review, Release and Filing of Information
- Answers and responds to incoming calls in a courteous and professional manner following Standards of Behavior.
- Responsible for assisting other departments in locating/printing electronic records for various reviews and/or studies.
- Responsible for completion of release of information requests for continuation of care, for patient requests, other department requests, i.e. Business Office, QI/UM, and/or after authorization for records, i.e. attorneys, insurance companies, etc.
Medical Record Analysis and Completion
- Responsible for analysis of al electronic medical records for outpatient services and timely completion of work queues. Analysis of records electronically includes verification that all documents are signed either manually or electronically; the testing/procedure ordered has results/documentation and orders; orders are current, dated, list testing to be performed, diagnosis and are signed by practitioner. Responsible to print unsigned e-sign report and co-signature/verbal order report to verify any electronic signatures needed on medical records and follow up with respective practitioner to get their documents e-signed.

- Practices and reflects knowledge of HIPAA, DIA, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
- Completes all annual education and competency requirements within the calendar year.
- Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
