We are seeking a strong candidate to join our team of medical providers, behavioral health providers and care coordinators who work collaboratively to assist individuals and families reach optimal health and wellness. As trusted, highly trained members of the community, Community Health Workers support individuals, families and the community as a whole in connecting to and navigating through the healthcare system. Additionally, Community Health Workers provide care coordination that includes assessment, screening, planning, implementation, monitoring, advocacy and linkage to available services and resources.
Essential job functions will include:
· Help process incoming and outgoing referrals
· Care coordination, case management and system navigation, including making referrals, facilitating transportation services and addressing barriers to services
· As part of the care team, provides culturally appropriate group and individual education on specific health related topics and works on special health education projects
· Advocates for individuals, families and communities with regard to basic needs and social determinants of health.
· Provides direct services (ex. care coordination, disease management education, health goal setting, etc.) to patients
· Documentation of encounters with patients in electronic medical record
· Builds relationships and collaboration with other community organizations.
· Uses data sets from our CCO to manage populations of patients with specific risk factors or needs.
· Assists in creating a positive and supportive work environment, enforces a safe workplace; establishes a culture of teamwork and communication, creates a workplace that promotes the organizational values of workplace diversity, equity, inclusion and actively promotes an environment respectful of living and working in a multicultural society.
· Encourages and empowers individuals and families to be active, engaged participants in their health.
· Helps patients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
· Conducts outreach (e.g., follows-up on health and social service encounters with individuals, families, and community groups; home visits to provide education, assessment, and social support; presents at local agencies and community events).
· Advocates for clinic quality improvements on behalf of observed/requested needs for community members.
· Spanish fluency highly preferred.
· Must have a valid CHW certification.
