Carter Clinical Services, pLLC is seeking a full-time Physical Therapist to join our team serving Hamilton County Hospital in Syracuse, Kansas. This position provides Physical Therapy services across multiple care settings, including outpatient, acute inpatient, and skilled/swing-bed care.
This is an excellent opportunity for a Physical Therapist who enjoys the variety and continuity of care that comes with working in a rural hospital setting. The Physical Therapist will evaluate and treat patients with a broad range of diagnoses and functional needs, helping patients improve mobility, function, independence, and overall quality of life. Duties & Responsibilities
- Provide Physical Therapy evaluation and treatment services in outpatient, acute inpatient, and skilled/swing-bed settings.
- Perform comprehensive patient evaluations and assessments appropriate to each patient's diagnosis, condition, and level of care.
- Establish individualized patient goals and develop effective plans of care.
- Implement safe, effective, evidence-based, and functionally relevant treatment interventions.
- Assist hospitalized and skilled-care patients in improving mobility, strength, balance, transfers, gait, functional independence, and safety in preparation for discharge or transition to the next level of care.
- Provide outpatient rehabilitation services for patients with orthopedic, neurologic, musculoskeletal, post-surgical, and other rehabilitation needs.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Collaborate with physicians, nursing staff, case management, and other members of the interdisciplinary care team regarding patient progress, discharge planning, equipment needs, and recommendations for continued care.
- Complete accurate and timely patient documentation and enter charges for services provided.
- Provide clinical supervision and direction for care delegated to technical and supportive personnel in accordance with applicable laws, regulations, and organizational policies.
- Communicate effectively with patients, families, healthcare professionals, and other members of the care team.
- Demonstrate professionalism and strong interpersonal skills, including effective written and verbal communication and the ability to navigate difficult situations appropriately.
Qualifications
- Education: Graduate of an accredited Physical Therapy program.
- Experience: A minimum of one year of Physical Therapy experience is preferred. Experience in outpatient, inpatient, skilled nursing, or other hospital-based rehabilitation settings is beneficial but not required.
- Licensure: Must hold or be eligible to obtain current licensure to practice Physical Therapy in the State of Kansas.
