Job Description
We are looking for a licensed Physical Therapist to provide skilled in-home therapy services. The PT will perform evaluations, create treatment plans, and deliver interventions that support clients in improving mobility, strength, balance, and overall functional independence.
Responsibilities
- Complete in-home physical therapy evaluations and reassessments.
- Develop and update individualized treatment plans based on client goals and functional needs.
- Provide skilled PT interventions such as gait training, balance training, therapeutic exercise, transfer training, and fall prevention.
- Educate clients and caregivers on exercises, mobility techniques, and safety measures.
- Maintain timely and accurate clinical documentation.
- Coordinate with the supervising nurse, agency staff, and other care team members.
- Follow all regulatory and agency standards for home health services.
Minimum Requirements
- Active Georgia Physical Therapy License (PT) in good standing.
- Current CPR certification.
- Reliable transportation to complete home visits.
- Ability to work independently in a home setting.
- Strong documentation and communication skills.
Minimum Qualifications
- At least one year of clinical experience (home health preferred but not required).
- Able to pass all required background checks.
Job Types: Full-time, Part-time, Contract
Pay: $50.00 - $70.00 per hour
