CarePlus TLC is seeking a dedicated, compassionate, CNA/Home Health Aid with a passion for serving others to join our team!

· Client is located in Rockford by the Meijer off 131 and 10 Mile Rd. Shifts are 3 days per week 3 hour shifts: either 8a-11a or 9a-12p
As a Home Health Aide/CNA at Care Plus TLC, you will play a vital role in assisting patients with daily living activities and providing essential health care support. This position is crucial in enhancing the quality of life for our clients while promoting their independence.
*Must have a valid driver's license
*Must own your own reliable vehicle
Responsibilities:
- Monitor patient’s condition and report changes/concerns to appropriate medical professionals and family members
- Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, grooming, etc.
- Perform light housekeeping duties to maintain a safe and clean-living space for clients.
- Provide companionship and emotional support to enhance patient well-being.
- Understands First Aid and Emergency Care protocols
- Complete electronic documentation while on shift
- Medication reminders
- Adhere to HIPAA regulations and maintain patient confidentiality.
Skills:
- Familiarity with Home Health Care, safety and sanitation standards and procedures
- Understanding of Confidentiality and Privacy Laws
- Excellent communication and interpersonal skills… able to communicate effectively with patients and their families.
- Knowledge of basic medical terminology
- Detailed oriented
- Ability to turn and reposition patients
- Ability to undertake a variety of diverse care tasks and work as a team member
- Compassionate and able to handle tough situations as needed
- 1 year experience as a caregiver is preferred.
- Ability to work effectively with individuals with developmental disabilities or dementia is desirable.
