Physician Assistant — Aesthetics & Wellness
Sage & Honey Wellness · Rancho Mirage, California Part-time · Base + commission · In person
Our philosophy
Our spirits don't age the way our bodies do.
Our bodies carry the wear and tear of a life well lived. Our joints ache. Our hair thins. Our energy shifts. Yet inside, many of us still feel like the same person we've always been.
We don't pursue wellness because we're chasing beauty or because we're afraid of getting older. We pursue it because we still feel like ourselves — and we want to keep the body that lets us live like it.
The body that carries us up the trail with friends, and out under the stars with a pack on. The body that jumps in the water instead of watching from the shore. The body that bends down to pick up a dog who's been waiting all day.
We believe the body should never become the reason someone stops living the life they love.
So our mission is simple: to give people back time in their own bodies — and a little more joy in every day.
None of us can turn back time. Giving any of it back sounds just as impossible — but we don't think it is. Every day someone spends sitting out, feeling like a stranger to themselves, is a day they don't get to use. Give those days back, and you've given them time. That's the impossible thing, and it's what this practice exists to do.
We are not another aesthetics clinic. Beauty is often the doorway, and we take that work seriously — but it isn't the point. The point is the person who books a neurotoxin appointment and leaves excited, looking forward to watching the crease between their brows soften over the next few days. The point is the patient who looks in the mirror a month after resurfacing and finds a familiar face there — who isn't counting lines, isn't reaching for more makeup, and isn't thinking about any of it by the time they're out the door. The point is the person whose energy comes back and who starts saying yes to things again. Every patient defines wellness differently, and we consider it a privilege to be part of that.
The position
We're looking for a Physician Assistant to be our daytime provider.
Our medical director sees patients evenings and weekends. Our patients need someone they can see during the day — for follow-ups, for treatments, for the consultation they couldn't fit in after work. That's the seat we're filling, and it comes with real independence once you're trained for it.
It's also a chance to practice the way you meant to when you started.
That means real appointment time. Consultations where you can actually listen. The room to use your clinical judgment and develop the treatment plan that's right for the patient — including when they want more and the answer is no. You'll build a panel of patients who come back for you — not for a coupon, not for a laser, for you.
You'll consult, assess, plan, and carry out the treatments. In your treatment room, you're the clinical authority. And because we're small and still new, the way we practice is not yet set in stone — your judgment will help shape our protocols and our standards.
You'll also practice alongside the physician who supervises you. Questions and hard cases are always welcome to be discussed. An open line of communication — a real clinical partnership with your supervising physician — is there whenever you need it.
If what you want is to be scheduled at fifteen-minute intervals and measured on units sold, we'll save you the application. If what you want is to do careful, conservative, beautiful work for people you know by name — keep reading.
Schedule and expected hours
Part-time, 16–24 hours per week, with a clear path to expanded hours and full-time as the practice grows. The schedule changes once you're trained.
During training — evenings and weekends. Your first 12 weeks are spent shadowing and then working beside our medical director during evening and weekend clinic. Nothing is thrown at you. You'll watch our protocols run, take over piece by piece, and treat with a physician in the room until you're both confident you don't need one.
After training — daytime. You'll take 2–3 daytime shifts a week, Wednesday through Sunday, 10:00 am to 6:00 pm, scheduled in advance. That's the whole point of this role: our medical director sees patients evenings and weekends, and our patients need a provider they can see during the day. Follow-ups, treatments, and consultations become yours to run.
- Occasional evening hours for patient events
- Flexibility to pick up additional shifts as the schedule fills
- No overnight call, ever
Duties and responsibilities
We are a growing practice and our protocols evolve as we learn. The list below describes the shape of the role rather than every task in it.
Scope of practice. All clinical activity is performed under a written Practice Agreement with our medical director, and under our standard operating procedures and written clinical protocols, consistent with the California Physician Assistant Practice Act and Medical Board guidance for medical spa services. You will be expected to know those boundaries precisely, work inside them without exception, and raise anything that sits near the edge rather than resolving it on your own.
Consultation and treatment planning
- Conduct unhurried consultations that start with what the patient actually wants back in their life, not with a menu
- Perform facial assessment, skin analysis, and full aesthetic evaluation
- Take relevant medical history, review medications and contraindications, and determine treatment eligibility
- Build treatment plans that sequence care over time and set honest, specific expectations about what a treatment can and can't do
- Decline or defer treatment when it isn't right for the patient
- Order, interpret, and act on labs where indicated
Aesthetic treatment
- Administer neuromodulators with an eye toward natural movement and expression
- Perform microneedling, PRP/PRF, chemical peels, and skin-health treatments
- Operate our energy-based device platform as credentialed and as permitted under your Practice Agreement — ablative fractional CO2 resurfacing (Lumenis AcuPulse), IPL and non-ablative fractional treatment (Lumenis Stellar M22), and RF microneedling with dynamic muscle stimulation (Lumenis triLift)
- Recommend medical-grade skincare and home regimens as an extension of the treatment plan
- Educate, never pressure
IV and wellness therapies
- Deliver IV hydration and nutrient infusion therapy, including patient selection and formulation review
- Administer vitamin and lipotropic injections
- Manage medical weight-management protocols, including GLP-1 therapy, intake, dosing, monitoring, and side-effect management
- Oversee peptide and adjunct wellness protocols within scope
- Provide the follow-up that makes these protocols actually work — check-ins, adjustments, and honest conversations about adherence
Safety and complication management
- Obtain and document informed consent for every treatment
- Maintain expert-level knowledge of facial anatomy, injection depth, and diffusion patterns
- Recognize and manage adverse events — including ptosis and unintended muscle weakness, post-laser burns, infection, prolonged erythema, and infusion reactions — per rehearsed written protocol
- Screen for and manage post-inflammatory hyperpigmentation risk across skin types, particularly with resurfacing and IPL
- Escalate to the medical director per agreement and per judgment
- Conduct post-treatment follow-up and manage outcomes that need revision
Documentation and clinical photography
- Chart completely and same-day, including treatment details, product, lot, and quantities
- Follow our photography protocol exactly: standardized angles, gridlines, animation series where indicated, device captures, and the save-immediately discipline that keeps our timestamps accurate
- Review and sign notes drafted by clinical support staff
- Secure written consent before any external use of patient images; de-identify where required
Practice building and stewardship
- Build and retain your own patient panel through outcomes and relationships
- Support membership and retention programs and treatment-series adherence
- Participate in social content and community education as you're comfortable — we won't make your face the marketing plan if that's not you
- Contribute to injectable and product purchasing decisions and vendor relationships
- Help develop new service lines where you bring specialized skill — if you're strong in something we don't yet offer, we want to hear about it
Protocols, compliance, and continuous improvement
- Learn our written protocols, follow them precisely, and help keep them current
- Uphold HIPAA, OSHA, infection control, and California Medical Board requirements in every part of the day
- Mentor and train clinical support staff
- Complete assigned training and manufacturer education; keep licensure, DEA, BLS, and ACLS current
What we're looking for
Required
- Current, unrestricted California Physician Assistant license
- NCCPA certification (PA-C) in good standing
- Master's degree from an ARC-PA accredited program
- Current DEA registration or eligibility
- Current BLS certification; ACLS preferred
- Willingness to enter and practice within a written Practice Agreement
- Weekend availability
Strongly preferred
- 2+ years of hands-on neuromodulator injecting experience
- Demonstrated proficiency with IV insertion and infusion therapy
- Experience with energy-based devices — CO2 resurfacing, IPL, or RF microneedling; Lumenis platform experience a plus
- Background in functional or integrative wellness, hormone therapy, or GLP-1 weight management
A note on fillers. Fillers aren't a focus for us. Our aesthetic work centers on neuromodulators, energy-based treatment, and skin health, and you don't need filler experience to do this job well. But if you're genuinely proficient with them, there's an opportunity here: we'd be glad to build a filler program around a provider we trust to do it conservatively and safely. Tell us about your experience and we'll talk about what that could look like.
New graduates — we're open to it. If you're newly certified and you want to learn this work properly, apply and tell us about it. You don't need aesthetics on your résumé yet. What we're looking for is someone who believes in our mission and our philosophy, wants to be trained, and takes feedback as coaching. Someone humble, motivated to keep building their skillset, and patient enough to do things exactly right before doing them quickly. Someone who helps the team — and who sweeps the floor when no one's looking. (Everyone sweeps, even our founder!) We care about this clinic and we care about our work, and we each do our part to take care of it — and of each other. We would rather train excellent judgment than correct bad habits.
If that's you, expect a longer supervised ramp, close daily oversight from our medical director, and a starting rate that reflects the training period — and expect us to teach you seriously in return.
And in any case
- A conservative hand and a good eye — you'd rather under-treat and bring someone back than overdo it once
- The confidence to tell a patient no, and the warmth to make them glad you did
- Careful, detail-exact work; our documentation and photography protocols leave no room for improvisation
- Discretion with sensitive information
- A yes mindset — you look for the way something can work
Compensation
Base: $55–$85 per hour, depending on experience. New graduates start at $50/hour during a supervised training period of 12 weeks, moving to the standard base on completion.
Commission: Earned on services performed and retail sold, calculated and paid quarterly. Full terms are provided with your offer.
Benefits: Health insurance is not offered at this time. We expect to add benefits as the practice grows.
Also included
- Malpractice insurance coverage
- Paid training on our protocols and systems
- Support toward CME, licensure, and DEA renewal as the practice grows
- Manufacturer training opportunities
- Treatment and product perks
- Direct mentorship and practice alongside the physician who supervises you
Our values and how we work
S*ervice · Accountability · Growth · Excellence Humility · Ownership · Nurture · Everyone · Y*es mindset
What that means day to day: feedback here isn't criticism, it's coaching. We value people who naturally ask themselves is this the best I can do? and how can I make this even better? Excellence isn't something we manage; it's a standard each person chooses.
And we think beyond ourselves. A clinic is a small society, and it only works when each of us holds its wellbeing alongside our own — looking after the room, the team, and the person next to you, not only your own patients. That care extends outward, to the community we're part of and to people who may never book an appointment with us.
Your work here will matter. You'll help shape the patient experience, write the protocols, and build the clinical culture of a practice that's just getting started. Your effort won't disappear inside a big organization — you'll see the impact of it, in the schedule, in the outcomes, and in the patients who come back and bring their friends.
We're building toward a world where no one stops living fully at any stage of life. That's a large thing to say out loud, and we know it. But it happens one patient at a time, in a treatment room, with a provider who took the time to get it right.
