Provide support coordinationcase management services to adults with developmental/intellectual disabilities. Assure that individuals receive appropriate services and assist them in accessing needed medical, psychiatric, educational, residential and other support services. Bachelor's degree in human services field preferredone-year experience working with the population required. Experience with assessmentservice plan development desirable. Must be computer literate. Salary is determined by educationexperience as it relates to the position. Full time, with benefits.
