2. Position Overview
Seeking a Contract Registered Nurse – Home Health (RN) to join our team.
- Schedule: Monday–Friday, 8-hour shifts (until patient visits are complete)
- Duration: Contract assignment
- On-call: Required as needed based on staffing and branch needs
3. Key Responsibilities
- Provide skilled nursing care in patients' homes.
- Complete patient assessments and develop care plans.
- Document care accurately using Homecare Homebase.
- Educate patients and families on medications and home care.
- Perform wound care, IV therapy, catheter care, and other nursing procedures.
- Coordinate care with physicians and the interdisciplinary team.
- Maintain compliance with OASIS documentation and home health standards.
4. Requirements
- Active Registered Nurse (RN) license.
- Minimum 2 years of Home Health RN experience.
- Experience with Homecare Homebase (HCHB).
- Minimum 1 year of OASIS documentation experience.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong communication, documentation, and organizational skills.
5. Education
- Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
- BSN preferred.
6. Certifications
- Active RN License.
- Current CPR Certification.
- Valid Driver's License.
7. Preferred Qualifications
- CHPN certification.
- Experience with venipuncture, IV therapy, wound care, and home health nursing.
- Ability to work independently and manage multiple patient visits.
Pay: $2,000.00 - $2,200.00 per week
